What is Aged Care and Dementia Care Support?

Aged Care and Dementia Care Support services are designed to help older adults live safely and independently for as long as possible. For individuals living with dementia, our services focus on managing cognitive decline, promoting memory support, and providing strategies for navigating daily tasks.

Our Occupational Therapists assess the needs of the individual and develop a tailored care plan that addresses their physical, emotional, and cognitive challenges.

At Sigma Therapies, we understand the unique needs of ageing adults and those living with dementia, offering comprehensive support to ensure they continue to enjoy a high quality of life.

How Our  Aged Care and Dementia Care Support Services Can Help You

Our services are designed to address the unique challenges faced by older adults and those living with dementia, including:

Supporting Daily Living Skills:
We work with individuals to maintain independence in personal care, mobility, and household tasks, providing strategies to overcome any physical or cognitive limitations.

Memory and Cognitive Support:
For those living with dementia, we implement memory aids, cognitive training, and structured routines to promote brain health and slow cognitive decline.

Adapting the Home for Safety:
We assess the home environment and recommend modifications such as grab rails, improved lighting, and reducing tripping hazards to ensure a safe and supportive living space.

Promoting Social Engagement:
We help older adults and individuals with dementia remain socially active, reducing isolation and improving emotional well-being through community participation and social interaction.

Carer Education and Support:
We provide carers and family members with training, resources, and strategies to support their loved ones, ensuring they can manage the challenges of dementia and ageing with confidence.

Emotional and Behavioural Support:
Dementia can lead to changes in mood and behaviour. Our Occupational Therapists provide strategies to manage anxiety, agitation, or depression, ensuring individuals feel safe and supported.

What’s Involved in Our Aged Care and Dementia Care Support Services?

Our care plans are highly personalised and developed to meet the specific needs of the individual. The process typically includes:

Initial Consultation:

We begin by discussing the needs, preferences, and challenges of the individual and their family or carers. This helps us understand the key areas where support is needed.

Assessment of Physical and Cognitive Abilities:

Our Occupational Therapists conduct a comprehensive assessment to evaluate physical mobility, strength, balance, and cognitive function. This helps identify areas where assistance may be required to support daily living or dementia care.

Home Safety Assessment:

We assess the home environment to ensure it is safe and accessible. If needed, we provide recommendations for modifications such as grab rails, non-slip flooring, and dementia-friendly designs to reduce confusion and accidents.

Cognitive and Memory Support Strategies:

For individuals living with dementia, we develop personalised cognitive support strategies, including memory aids, routines, and environmental adaptations to help with orientation and memory retention.

Personalised Care Plan:

Based on the assessment, we create a tailored care plan that addresses the individual’s physical, cognitive, and emotional needs. This plan may include recommendations for assistive technology, home modifications, and strategies for maintaining independence.

Ongoing Monitoring and Support:

We provide continuous support to monitor progress, adjust the care plan as needed, and ensure that the individual’s needs are being met over time.

Is Aged Care and Dementia Care Support Covered by Medicare?

Aged Care and Dementia Care Support services may be funded through government programs such as the Aged Care Assessment Team (ACAT) for eligible individuals, especially those needing access to home care packages or residential aged care.

Medicare does not generally cover these services, but some private health insurance plans may offer rebates for Occupational Therapy support. NDIS funding may also be available for eligible participants with dementia who are under 65.

We recommend discussing your funding options with your healthcare provider, NDIS planner, or support coordinator to explore available options.
